Dr. Jay Chanmugam graduated from the University of Texas at Austin and went on to earn his medical degree from the University of North Texas Health Science Center in Fort Worth, TX. He interned at Saint Vincent Mercy Hospital in Toledo, OH, and he performed his residency in emergency medicine with Pennsylvania State University’s York Hospital at Hershey, PA. He is certified by the American Board of Emergency Medicine (ABEM). He is also certified by the American Academy of Addiction Psychiatry and the Substance Abuse and Mental Health Services Administration as a provider for Suboxone therapy.
Dr. Sylvia Collazo found her passion for medicine as a young child in Puerto Rico while watching her grandmother, who was a nurse, care for sick people in their homes. Her grandmother helped those in the community who had no money, giving them injections and alleviating their pains and sickness while asking for nothing in return. It was her grandmother’s giving spirit that taught Dr. Collazo compassion and respect for her fellow man. This compassion and respect ultimately fueled her desire to become a physician. Dr. Collazo did her residency at The Medical Center in Aguadilla, Puerto Rico and The Medical Center at Río Piedras, Puerto Rico. In 1977, she graduated from Central Del Este Medicine School in the Dominican Republic with her degree in medicine.
